The Trenord train was heading to Milan from Cremona, in northern Italy. The cause of the crash is not yet clear. It happened just before 07:00 (06:00 GMT). A problem with the coupling between carriages is being investigated as a possible cause of the accident.
Pictures show the twisted wreckage of two carriages at the scene. The train had been heading for Milan's Porta Garibaldi station. The Trenord train was heading to Milan's Porta Garibaldi station from Cremona when the incident happened at around 07:00 (06:00 GMT).
Two helicopters have been deployed to the rescue operation, newspaper Corriere Della Sera reports (in Italian). Some of the injured are receiving first aid in a nearby field.
A witness told Repubblica newspaper that the train "began to tremble" before the accident.
Milan police chief Marcello Cardona said: "We have identified a failure between wagons, but all the investigations are still underway."
