During his murder trial, prosecutors described Saadallah’s Thames Valley Police also failed to notify the probation service about the call they’d had from Saadallah’s brother, the inquest heard.
attack as "ruthless and lethal". An urgent, out-of-hours recall could have been actioned by the probation service - had they been given the information.
At around 18:45 BST witnesses saw a lone knifeman target a group of people in Forbury Gardens near Reading town centre. This was a missed opportunity to get Saadallah off the streets and back into prison, the day before he went into Forbury Gardens in Reading, killed three people, injured three others and attempted to kill many more.
Friends James Furlong, 36, Dr David Wails, 49, and Joseph Ritchie-Bennett, 39, were fatally stabbed. Ultimately, this man wasn’t just "known" to the authorities - he was "very well known".
Three others - Stephen Young, Patrick Edwards and Nishit Nisudan - were also injured. As a result of all of the many conclusions read out by Sir Adrian Fulford, in his role as judge coroner, he will be issuing numerous Prevention of Future Death notices to government ministers, government departments and many other state agencies.
Saadallah shouted "Allahu Akbar [God is greatest]" as he attacked and again as he ran away, throwing away the 8in (20cm) knife as he was pursued by an off-duty police officer. The conclusions are still continuing.
At 18:56 the first emergency call was made to Thames Valley Police.
Chief Constable John Campbell said officers arrested a man within five minutes of the call.
