BBC Scotland has uncovered new evidence about what is going wrong in our hospitals.

<a href="http://www.bbc.co.uk/programmes/b01p6w7y" >BBC Scotland Investigates: How safe is your hospital?</a> asks whether we can trust the NHS to do the right thing when mistakes are made.

After a freedom of information request, we were given 345 critical incident reports, including 105 deaths. They are all published here.
