Emergency services were called to the crash near the Cwrt Bleddyn Hotel, Llangybi, at about 19:25 BST. The alarm was raised after the crash near the Cwrt Bleddyn Hotel, Llangybi, on the Usk to Caerleon road at about 19:25 BST on Tuesday.
South Wales Fire and Rescue Service said three people were taken to hospital by road in ambulances. The fourth was airlifted. Gwent Police said four motorcyclists were taken to hospital. One has possible life-threatening injuries.
Fire crews from Usk and Malpas in Newport helped the ambulance service deal with the collision. The road was closed for several hours while officers carried out an investigation.
