Ferry operator Northlink said the captain of the Hrossey was notified of the incident at about 04:00.
They said he immediately turned his vessel to begin a search for the passenger.
The MV Hrossey left Kirkwall at 00:25 and had been due to arrive in Lerwick at 07:30.
It had 172 passengers on board, including 21 children.
