Allison Heathcote, from Felixstowe, Suffolk, is in a critical but stable condition at the Queen Elizabeth Hospital in Birmingham.
A spokesman for University Hospitals Birmingham NHS Foundation Trust, which runs the hospital, said the 48-year-old had undergone five hours of surgery after suffering multiple gunshot wounds - three to her torso and two to her right arm and shoulder.
Mrs Heathcote had already had surgery in Tunisia on Saturday and was returned to the UK by the RAF.
Cheryl Mellor
Cheryl Mellor, 55, from Bodmin, Cornwall, was left with gunshot wounds to an arm and leg.
She was airlifted out of the country and brought back to the UK by the RAF.
Her husband Stephen Mellor was among those killed.
Mrs Mellor told the BBC that when the shooting started the couple huddled together, saying "I love you".
Tony and Christine Callaghan
Among the injured are Tony Callaghan, 52, and his wife Christine, 62, from Norfolk, who were on holiday.
Mr Callaghan, who was shot in the knee, is a member of staff at Norfolk police. Mrs Callaghan was shot in the leg.
Their injuries have required hospital treatment but are not life threatening.
BBC correspondent Mark Lowen met the couple in hospital and saw the bullet holes in Christine's handbag.

Gas engineer Mathew James from Trehafod, near Pontypridd, was shot three times and used his body as a shield to protect his fiancee Saera Wilson, 26.
He was hit on the shoulder, chest and hip.
The 30-year-old, who later suffered a heart attack, has since returned to the UK and is now "stable and comfortable" in hospital.
'Like a postman'
He told the Sunday Mirror he "reacted without thinking" when the shooting began and turned his fiancee around before falling on her to protect her.
Mr James said he was then shot twice more in his stomach and chest and told Saera he was going to die before imploring her to leave and tell their children he loved them.
"I didn't want her to go but I knew they needed to have one parent at least," he said.
Mr James also spoke about seeing gunman Seifeddine Rezgui: "There was no emotion in his face.
"He looked like a postman going about his business but instead of delivering letters he was strolling along shooting people."
'Outstanding bravery'
In a statement, the couple thanked medical staff and all those who had sent messages of support.
His plight has touched fellow gas engineers, who launched a fundraising drive in honour of the father-of-two while he recovers from his injuries. Donations on the page have surpassed £15,000.
And his bravery was also recognised by the First Minister of Wales, Carwyn Jones, who tweeted: "I would like to commend Matthew James' instinctive and outstanding act of bravery #Tunisia."

The couple, from Coventry, are recovering from shrapnel injuries.
Mr Thompson told Sky News they fled and hid indoors when the attack started.
"We managed to get the room barricaded, got down low and just hid," he said.
His girlfriend said she got separated from her partner and hid in the staff toilets with another woman and her son.
"We locked ourselves in and hoped for the best," she said.
