A fifth man injured in the crash has been taken to hospital.
A West Midlands Ambulance Service spokesman said the crash involved one car carrying four men and the other a lone male driver.
The lone driver and three men from the other car were confirmed dead at the scene.
The Highways Agency spokesman said one of the cars was travelling northbound on the southbound carriageway.
